北京中日医院杨文英教授团队的一项研究摘要2015年第75届美国糖尿病协会(ADA)科学年会上发布。该研究调查了中国
目的:既往研究已观察到中国人群的糖尿病发病率具有性别差异,但在代谢综合征人群中糖尿病发病率是否具有性别差异尚不清楚。
方法:研究人员前瞻性调查了159例男性、301例女性代谢综合征患者中性别差异与糖尿病发病率之间的相关性。采用Cox比例风险回归模型对相关性进行评估。
结果:在平均随访2.5年后,共有15例患者出现糖尿病,其中9例男性、6例女性。校正基线时年龄、受教育程度、职业、治疗药物、腰围、血压、HbA1c、甘油三酯、低密度脂蛋白等多种混杂因素后,男性糖尿病发病风险较女性增加了6.49倍(HR 6.49,95% CI 1.51-28.0)。甘油三酯水平每增加1 mmol/L,所有患者的的糖尿病风险增加31.4%(95% CI 1.02-1.70),其中男性患者糖尿病风险增加65.0%(95% CI 1.12-2.43),女性患者增加37.0%(95% CI 0.45-4.15)。
结论:该研究提示对于代谢综合征患者,男性的糖尿病发病风险高于与女性。甘油三酯水平可能是导致男性代谢综合征患者糖尿病风险增加的重要因素。

| Abstract: | Background Sex differences in the prevalence of diabetes mellitus were observed in China. But whether there is difference between sexes in the incidence of diabetes mellitus did not describe before. Methods We prospectively investigated the association between sexes and incidence of diabetes mellitus among 159 male and 301 female subjects with metabolic syndrome. Cox proportional hazards regression models were used to estimate this association. Results During a mean follow-up of 2.5 years, 15 incident cases (9 in male, 6 in female) of diabetes mellitus were identified. The multivariable-adjusted (age, education, occupation, medicine treatment, waist circumference, blood pressure, HbA1c, triglyceride and low density lipoprotein cholesterol at baseline) hazard ratio (HR) for incidence of diabetes mellitus associated with sex was 6.49 (95% confidence interval [CI] 1.51-28.0). There was a 31.4% increase of the risk of diabetes mellitus with 1 mmol/l increase of triglyceride (95% CI 1.02-1.70) in all subjects, with 65.0% increase in male (95% CI 1.12-2.43) and 37.0% increase in female (95% CI 0.45-4.15), respectively. Conclusions The current study indicates the risk of diabetes mellitus is higher in male than female subjects with metabolic syndrome. Triglyceride may contribute to the high risk in male. |
